Under Your shop, Forms, your trade's templates are one tap away, and More templates shows the rest. Edit the questions, move them up or down with the arrows beside each to change the order they are asked in, choose whether a form is asked on the first visit (and again after it lapses), every visit, or only when you ask, and which services it is for. Health and consent forms end with the agreement wording, a retention line and a typed name.
When a customer books one of those services, the form appears on the booking page under Before you come, after they type their email, and is answered before the booking is made. If they booked by phone, or skipped it, their booking page asks for it and the confirmation email says so. Inside the validity window they are not asked twice.
In the diary, each booking's card shows every form done (press it for the answers) or not yet. The customer's profile has a Forms section with a row for each form you ask: open one to see every time they completed it, each opening as the answers with the agreement, with a PDF to download.
If a customer has already done a form some other way, for example a patch test you did in person before you moved to FullDiary, open that form on their profile and press Don't require for this customer. Pick the date it is not needed until and, if you like, a reason. Until that date they are not asked for it on any booking, and the diary card says it is not needed; after it they are asked as normal. Ask again takes it off straight away. Sensitive answers are never exported any other way. Forms are free on every plan; the switch is at the top of Your shop, Forms.
